Appointment Information

One week before your first appointment, we’ll send you an online questionnaire to fill out through MyChart. We ask that you fill this out at home. If you can’t complete it at home, please arrive one hour before your scheduled appointment so you can complete the forms at the office.

On the day of the appointment, please allow time for parking and registration. We ask patients to arrive 30 minutes early.

Please bring your medical records, imaging CDs, a current insurance card, a method of payment (we accept checks or debit/credit cards only), and a complete list of current medications and dosages.

If you need to cancel an appointment, please notify us at least 24 hours in advance so we can accommodate other patients waiting to schedule an appointment.

Getting Here

While there is self-parking available, we recommend valet parking, available right at the front entrance. Once inside the building, take the elevator in the front lobby up to the seventh floor.
