At UT Southwestern Medical Center, our house calls program – which we call Care of the Vulnerable Elderly (COVE) – provides comprehensive care for older individuals in the comfort of their own homes.

We understand the importance of building long-term, trusting relationships with both our patients and their family caregivers.

Our interdisciplinary team approach ensures that every aspect of our patients’ health needs is addressed with expertise and compassion.

From primary care to urgent care, we are here, when and where you need us most.

Who Is Eligible?

To qualify for our house calls program, a person must:

  • Be age 65 or older
  • Reside within 10 miles of UT Southwestern Medical Center (ZIP code 75390)
  • Meet any of the following criteria:
    • Unable to visit the primary physician's office due to memory or mobility impairment
    • Experiencing severe functional impairments, disabilities due to multiple chronic conditions

An Interdisciplinary Team for Quality Outcomes

Our team comprises dedicated professionals committed to providing personalized care:

  • Physicians: Lead the evaluation of all new patients and develop tailored care plans
  • Advanced practice providers: Specialized geriatric nurse practitioners provide ongoing care between physician visits, ensuring continuity and effectiveness of the care plan
  • Licensed clinical social workers: Offer supportive services, including counseling, education, and assistance with navigating resources
  • Nurses: Available during work hours for care coordination, triage, and addressing clinical concerns
  • Clinical administrators: Screen patients for program eligibility, handle scheduling, and facilitate referrals
