Appointment Information
We want to make your patient experience as easy as possible.
One week before your first appointment, we’ll send you an online questionnaire to fill out through MyChart. We ask that you fill this out at home to make your check-in process faster. If you can’t complete it at home, please arrive one hour before your scheduled appointment, so you can complete the forms at the office.
On the day of the appointment, please arrive 30 minutes early to allow time for parking and registration.
Please bring your:
- Medical records
- Imaging CDs
- Current insurance card
- Method of payment (checks or debit/credit cards only)
- Complete list of your current medications and dosages
To stay in touch easily, we recommend you sign up with MyChart, which allows you to communicate directly with doctors, request prescription refills, update contact information, view test results, and request clinic appointments. Sign in to or sign up for MyChart.
Canceling an appointment
If you need to cancel your appointment, please notify the center at least 48 hours in advance, so that we can accommodate patients waiting to schedule a visit.
Getting here
You may self-park for free with a voucher or use the valet parking service that is available at the front entrance for your convenience for a fee. Once inside the building, take the elevator in the front lobby to the seventh floor.
