Welcome to UT Southwestern Executive Health. We are pleased you have chosen UT Southwestern for your wellness assessment.

Our goal is to offer world-class, customized preventive health care based on your medical history. We combine advanced technology with traditional clinic care to provide a comprehensive annual assessment including lab work, imaging, a physical exam, a fitness assessment, and nutrition consultation – all tailored to detect early risk factors, promote a healthy lifestyle, inspire wellness goals, and support self-management of your health. You will receive a report from the Executive Health care team following your visit. The report will include test results and personalized recommendations to review and discuss with your primary care provider, so you can optimize your cognitive and physical performance.

Your Executive Health assessment will ideally include three parts: an online questionnaire to gather information about your health history, a virtual care pre-visit to develop a plan for your assessment, and the in-person physical assessment. Once you arrive at our clinic, you will be escorted to a private suite designed for a comfortable, yet efficient experience. You will receive a detailed copy of your schedule for the day, which includes a catered breakfast and lunch. Our staff will be with you throughout the visit to facilitate each step of your assessment and address any needs.

MyChart Registration

MyChart is a secure online tool for patients. Through MyChart you can communicate with your provider along with other benefits. If you have not signed up for MyChart, we urge you to do so. UTSWMyCare Mobile App

Our patient care mobile app – UTSWMyCare – helps you easily stay connected to your provider in a secure and safe environment. With the app, you have 24/7 access to the resources you need to manage your health – all in one place and all accessible with your smartphone. Key features include provider and location searches, shuttle services, campus wayfinding and more. MyChart is also included in the UTSWMyCare app, allowing you to do all the things you normally do on MyChart. Visit UTSWMyCare to learn more. Preparing for your Virtual Care Visit

Prior to your in-person Executive Health physical, you will have the opportunity to meet virtually with the clinic nurse and physician for a 30- to 60-minute virtual care appointment. UT Southwestern video visits allow you to connect with your health care provider on your own device, from any location. Your video visit can be conducted on a smartphone, tablet, or computer. You will connect to the visit through your MyChart account and our videoconferencing platform. To complete your video visit, you will need the following:

  1. An active MyChart account. Please contact the clinic where your appointment is scheduled if you are not certain about your account status.
  2. A compatible device, such as a smartphone (iOS, Android), tablet (iOS, Android), desktop, or laptop with an attached or built-in camera and microphone.
  3. An internet connection with adequate bandwidth.

On the day of your scheduled video visit:

  1. Log in to your MyChart account through your preferred device within 30 minutes prior to your scheduled appointment time.
  2. Select the "Appointments" icon.
  3. Select the appropriate scheduled video visit.
  4. Select and complete the “Pre-Check-In” process (if you have not previously done so).
  5. Select the "Begin Video Visit" icon.
  6. The videoconferencing platform will launch.
  7. Enable the microphone and video functionality.
  8. You will enter the virtual waiting room until your health care provider joins.

Preparing for your Executive Health Physical

  • Please arrive at your scheduled in-person appointment time to check in. Your visit will be six to eight hours long, so please block out that amount of time in your calendar.
  • Please bring your photo ID and insurance card(s), even if you are with a contracting company. We will scan these documents into your medical record.
  • Fasting for labs: Do not eat or drink anything except water after 8 p.m., the evening before your appointment. Drink plenty of water to avoid dehydration. Please continue to take your regular medications.
  • Do not exercise the morning of your visit, as this may interfere with metabolic testing.
  • Your Executive Health physical will consist of multiple consultations, including physical activity. We recommend you arrive in casual clothing (activewear), a sports bra with no metal (for women), and athletic shoes. You will have access to a shower. We recommend bringing a change of clothes, nonslip shower shoes and basic toiletries.
  • Please bring or wear your prescription glasses, contact lenses or hearing aids, if applicable.

Frequently Asked Questions

How long will my visit be?

Your assessment visit is comprehensive and will last six to eight hours. Please plan to be here most of the day.

Where should I park?

We recommend using our complimentary valet service located at the entrance to the clinic building.

What services are included in my visit?

Executive Health core services include a physical exam, lab tests, diagnostic imaging, exercise tolerance testing, nutrition and physical therapy consultations, and basic vision and hearing screenings. Our care team will determine what services are appropriate for you based on your personal and family history.

Can I bring a visitor to my appointment?

Yes, you are welcome to bring one guest to your visit. We request that all guests be at least 18 years of age. A private guest suite with Wi-Fi will be available for the duration of your visit.

How do I sign up for the online patient portal, MyChart?

You will receive a MyChart activation code when you schedule your appointment. Please visit our UTSW MyChart website to sign up or get more information or contact the MyChart Help Desk at 214-648-8888.

What should I wear for my visit?

Please wear comfortable, nonrestrictive clothing and athletic shoes. Women should wear a sports bra that does not contain metal. You are welcome to bring a change of clothes for after the physical fitness portion of the visit.

Do I need to fast for my labs?

Please do not eat or drink anything except water after 8 p.m. the night before your visit. Please continue your regular medications and drink plenty of water.

Can I exercise before my visit?

Please do not exercise the morning of your appointment, as this may interfere with metabolic testing.

Will I need my glasses?

Please wear or bring your prescription eyeglasses, contact lenses or hearing aids. If you normally use reading glasses, you may bring those as well.

Can I take a shower after the exercise portion of my visit?

Yes. You will have access to a shower. We will provide a robe, towels, body wash and shampoo. Please bring a change of clothes, basic toiletries, and nonslip shower shoes.
