Appointments at the Hepatology and Liver Transplant Clinic in Fort Worth

New Patients

All new patients must complete the Digestive and Liver New Patient Package.

On the day of the appointment, please arrive 30 minutes early to allow time for parking and registration. We will do our best to see you at your scheduled appointment time.

Initial Visit Checklist

Please bring the following items to your appointment:

  • Completed new patient forms
  • Recent test results or any medical records related to your visit
  • List of current medications and supplements
  • Name and contact information of any healthcare provider with whom our physician may need to speak
  • Driver’s license or other approved government picture ID
  • Insurance card
  • Method of payment – credit or debit card only 

To facilitate ongoing communication, we recommend patients enroll in MyChart, which allows patients to communicate directly with UTSW providers, update contact information, complete visit questionnaires, and request clinic appointments. Sign in or sign up for MyChart.

Canceling an Appointment

If you need to cancel an appointment, please call the clinic or cancel via MyChart at least 24 hours in advance so that we can accommodate other patients who might be waiting for an appointment. 

Should you cancel your appointment with less than 24 hours’ notice, you will be considered a “no show.” After three “no shows,” you will not be allowed to reschedule.
