Welcome to the UT Southwestern COVID-19 Vaccine Registration portal.

As of Monday, March 29, all adult Texans now qualify for vaccination, regardless of health status or occupation.

If you are not yet registered as a UT Southwestern patient in MyChart – our secure, online health management tool – we ask that you please complete the short questionnaire below to start the process for booking an appointment.

We need to collect some basic information so we know how to contact you regarding your vaccine and how to advise you based on your health background. We have taken all necessary safeguards to protect the privacy of your personal information.

What happens next?

COVID-19 vaccinations are by appointment only. After completing the registration survey, you will be sent a MyChart activation code. You should receive a message from mychart@utsouthwestern.edu within a couple days. If you do not see that message, please look in your spam folder. Once you register and log in to MyChart, you can search for available vaccination appointments.

How soon can I get vaccinated?

We are fortunate to now have a more stable supply of vaccine, increasing the speed with which we vaccinate individuals, oftentimes within days of signing up. Once 10,000 people submit their names, we then briefly pause registration and efficiently vaccinate those who just registered.

Answers to frequently asked questions can be found on our COVID-19 Vaccination webpage. We encourage you to visit that page often as information is constantly updated. There are also several educational resources and tip sheets available for your reference.

Additionally, our UT Southwestern COVID-19 Vaccination Information Line is live weekdays from 8 a.m. to 5 p.m., excluding holidays. You may reach an agent at 214-633-2021. We appreciate your patience as we are experiencing extremely high call volume. Please look for answers to your questions on our website first.
