Dr. Melanie Hafford

Meet Dr. Hafford

Melanie Lynette Hafford, M.D., is the Director of Bariatric Surgery at UT Southwestern’s Frisco campus, and she is certified by the American Board of Surgery and the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program. She specializes in laparoscopic bariatric surgery, foregut surgery, and minimally invasive general surgery.

Meet Dr. Chilakamarry

Sitaram Chilakamarry, M.D., specializes in colon and rectal cancers, cancers in the young, robotic surgery, and treatment of inflammatory bowel diseases. He is dedicated to helping patients with all forms of colorectal problems achieve their health goals, including those with pelvic floor disorders, colon and rectal cancers, diverticulitis, inflammatory bowel disease, and anorectal pathologies.

Dr. Ana Islam

Meet Dr. Islam

Ana Islam, M.D., is an endocrine surgeon specializing in disorders of the thyroid, parathyroid, and adrenal glands. She is board-certified in general surgery and her research has focused on recurrent kidney stones after successful parathyroidectomy, laparoscopic versus robotic adrenalectomy, excessive weight gain after pancreas transplantation, and persistence of de novo donor-specific antibodies after lung transplantation.

Dr. Gerardo Gonzalez-Guardiola

Meet Dr. Gonzalez-Guardiola

Gerardo Gonzalez-Guardiola, M.D., specializes in vascular surgery, with a focus on complex aortic surgery, aortic graft infection, cerebrovascular revascularization, and limb salvage. He completed a residency in general surgery and received advanced training in vascular and endovascular surgery. He serves on the National Surgical Quality Improvement Program (NSQIP) Initiative for Decreasing Pneumonia and Ventilator Over 48 Hours Task Force and joined the UT Southwestern faculty in 2021.

Preparing for Your Appointment

To make the most of your time with the doctor, prepare a list of questions ahead of time and bring it to your appointment.

On the day of the appointment, please arrive 20 minutes early to allow time for parking and registration. We do our best to see patients at their scheduled appointment time. 

Please bring the following items:

  • Current insurance card
  • Method of payment
  • Current medications
  • Completed paperwork, if you received forms before the visit

To facilitate ongoing communication, we recommend patients sign up with MyChart, which allows patients to communicate directly with doctors, request prescription refills, update contact information, view test results, and request clinic appointments. Sign in to or sign up for MyChart.

Canceling an Appointment

If you need to cancel your appointment, please notify the center at least 48 hours in advance so that we can accommodate other patients waiting to schedule an appointment.

Prescriptions and Refills

It is very important that you do not run out of your medication. Be sure that you have sufficient refills to last until your next appointment with your provider. We are happy to provide you with a written prescription upon request.

If you run out of refills, please call your pharmacy and your pharmacist will fax a refill request to us. Please allow us 24 to 48 hours to process refill requests.
