Dr. Melanie Hafford

Meet Dr. Hafford

Melanie Lynette Hafford, M.D., is the Director of Bariatric Surgery at UT Southwestern’s Frisco campus and she is certified by the American Board of Surgery and the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program. She specializes in laparoscopic bariatric surgery, foregut surgery, and minimally invasive general surgery.

Eric Michael Bass, M.D., is a Fellow of the American College of Surgeons and of the American Society of Colon and Rectal Surgeons. He specializes in robotic surgery – specifically, colon and rectal resections for benign and malignant disease, trans-anal minimally invasive surgery (TAMIS) resection of benign rectal tumors, and colonoscopy.

Meet Dr. Farkas

Linda M. Farkas, M.D., is certified by the American Board of Surgery and the American Board of Colon and Rectal Surgery, and she specializes in colorectal surgery. Dr. Farkas’s clinical interests include assessing risk for hereditary colon disorders and performing open, laparoscopic, robotic, and transanal colorectal surgery techniques.

Dr. Ana Islam

Meet Dr. Islam

Ana Islam, M.D., is an endocrine surgeon specializing in disorders of the thyroid, parathyroid, and adrenal glands. She is board-certified in general surgery and her research has focused on recurrent kidney stones after successful parathyroidectomy, laparoscopic versus robotic adrenalectomy, excessive weight gain after pancreas transplantation, and persistence of de novo donor-specific antibodies after lung transplantation.

Meet Dr. Kirkwood

Melissa Kirkwood, M.D., is Chief of Vascular Surgery. She specializes in endovascular therapies and lower-extremity bypass aimed at treating peripheral vascular disease (PVD) and salvaging limbs. She is an international expert in radiation safety and her research interests include radiation dose control, iliac artery aneurysms, and simultaneous thoracic and abdominal endovascular aortic repair.

Preparing for Your Appointment

To make the most of your time with the doctor, prepare a list of questions ahead of time and bring it to your appointment.

On the day of the appointment, please arrive 20 minutes early to allow time for parking and registration. We do our best to see patients at their scheduled appointment time. 

Please bring the following items:

  • Current insurance card
  • Method of payment
  • Current medications
  • Completed paperwork, if you received forms before the visit

To facilitate ongoing communication, we recommend patients sign up with MyChart, which allows patients to communicate directly with doctors, request prescription refills, update contact information, view test results, and request clinic appointments. Sign in to or sign up for MyChart.

Canceling an Appointment

If you need to cancel your appointment, please notify the center at least 48 hours in advance so that we can accommodate other patients waiting to schedule an appointment.

Prescriptions and Refills

It is very important that you do not run out of your medication. Be sure that you have sufficient refills to last until your next appointment with your provider. We are happy to provide you with a written prescription upon request.

If you run out of refills, please call your pharmacy and your pharmacist will fax a refill request to us. Please allow us 24 to 48 hours to process refill requests.
