If you wear contact lenses, you'll be asked to remove them prior to the assessment. They can be re-inserted following the exam. Please be sure to bring your contact lens solution and a case.

What to Bring to Your Dallas Vision Care Appointment

• Current insurance information
• Government-issued ID
• Method of payment (credit or debit card)
• A list of your current prescribed medications and over-the-counter (nonprescription) drugs, vitamins, or supplements
• A list of known drug allergies (if applicable)
• Relevant past medical records
• Completed paperwork, if you received forms before the visit
• A list of questions for the specialist and something to take notes with, if applicable

Cancelling an Appointment at the Dallas Laser Center

If you need to cancel an appointment, please call the clinic at 214-645-2027 24 to 48 hours in advance so that we can accommodate other patients who might be waiting for an appointment.
