The UT Southwestern Simmons Cancer Center Oncology Primary Care and Survivorship Clinic provides comprehensive, coordinated medical and survivorship care tailored to cancer survivors.
Our full-service clinic offers a broad spectrum of health services, ranging from preventive care to chronic disease management. We help cancer survivors at any stage of treatment improve their quality of life by:
- Optimizing control of chronic medical conditions
- Reducing modifiable risks
- Assessing for recurrence of cancer
- Addressing the long-term or late effects from cancer treatment
Team-Based Care
The clinic is staffed by primary care clinicians (physicians, physician assistants, and nurse practitioners) who work in coordination with oncology care teams and facilitate referrals to specialists and supportive care as needed. We have locations in Dallas and Fort Worth.

How to Prepare for an Appointment
On the day of the appointment, please arrive early to allow time for parking and registration.
Please bring the following items to your appointment:
- Current insurance card
- Driver’s license or other approved government picture ID
- Method of payment – credit or debit card only
- List of current medications
- Relevant past medical records, if available
- Completed paperwork, if you received forms before the visit
