The UT Southwestern Simmons Cancer Center Oncology Primary Care and Survivorship Clinic at Moncrief Cancer Institute helps cancer survivors at any stage of treatment improve their quality of life. Our services range from preventive care to chronic disease management.
We focus on controlling chronic medical conditions, reducing modifiable risks, and addressing the long-term or late effects from cancer treatment. We also assess for recurrence of cancer.
Team-Based Care
Our primary care clinicians (physicians, physician assistants, and nurse practitioners) work closely with oncology care teams and facilitate referrals to specialists and supportive care as needed. We also have a location in Dallas.

- Management of chronic health conditions, such as abnormal cholesterol, diabetes, heart disease, and hypertension
- Promotion of healthy lifestyles (diet, exercise, and smoking cessation)
- Care for acute illness
- Screenings for mental health and well-being
- Assessment for recurrence of cancer and screening for secondary cancers

How to Prepare for an Appointment
On the day of the appointment, please arrive early to allow time for parking and registration.
Please bring the following items to your appointment:
- Current insurance card
- Driver’s license or other approved government picture ID
- Method of payment – credit or debit card only
- List of current medications
- Relevant past medical records, if available
- Completed paperwork, if you received forms before the visit
