- Annual physicals, including sports physicals, well-child exams, and well-woman exams

- Chronic disease management for diabetes, hypertension, and hyperlipidemia

UT Southwestern’s Family Medicine - Primary Care Clinic has received Patient-Centered Medical Home Level 3 Accreditation. Level 3 Accreditation is the highest ranking from the National Committee on Quality Assurance (NCQA). Our Primary Care Clinic, located at 5939 Harry Hines Boulevard in Dallas, has been recognized for delivering this model of care that emphasizes high-quality care coordination by putting patients at the forefront.
