Requesting Your Medical Records
UT
Southwestern Medical Center provides release of information services for
patients who request medical records from UT Southwestern’s hospitals or
clinics.
Patient
medical records are available Monday through Friday, from 8 a.m. to 5 p.m., in
adherence with all state and federal regulations for the release of
information.
How to Request Medical Records
Patients
who want a complete copy of their medical records, radiology images, and/or pathology material must complete an Authorization to Disclose Protected Health
Information (en español) form (PDFs open in a new window). Patients
can also contact Health Information Management at 214-645-3030 (select option 1, then option 1 again), and a copy will be mailed. View detailed instructions on completing the medical request form (PDF).
Please note that there is a fee
for processing all medical records requests. However, there is no charge to
have records sent directly to another health care facility or physician for
continuation of care. Please visit the Office of Civil Rights for frequently asked questions on
access and allowable fees.
Obtaining the Records of Incapacitated
or Deceased Persons
To obtain
medical records for incapacitated patients, the authorized individual must
provide a valid copy of the patient’s Medical Power of Attorney (PDF), which states he or she has
permission to access the patient’s health information. When requesting records
for a deceased patient, a death certificate that identifies the requesting
party on the death certificate and the Executor/Administrator of the Estate or
Last Will and Testament/Testamentary documents are required.
